Santa Rita Home Health has created this Public Disclosure Statement in order to demonstrate the terms and conditions of our commitment to all interested parties.

Our information and dissemination practices pertaining to Public Disclosure can be found on our website and in our Patient Orientation Handbook.

Anyone has the right to request information about the ownership group and our management company. Patients, their representatives and interested parties may, upon request, receive information pertaining to the ownership group as well as to the Mission and practices of our Home Health Agency.

Santa Rita Home Health is owned and operated by Santa Rita Home Health LLC, whose past and present interest has been to provide excellent care to our home health patients and family members and to operate our business using the highest ethical practices.

Any patient or interested parties have the right to obtain information on licensing, credentialing and the services regarding our home health agency.

The Santa Rita Home Health Mission, Vision, Purpose and Values Statements can be found in all of our informational brochures, the Patient Orientation Handbook and on our website. The Santa Rita Home Health Mission, Vision, Purpose and Values is extended to our patients and family members, our vendors, our private partners (physicians, hospitals, skilled nursing facilities, assisted living facilities) and to every governmental, civic, religious and voluntary agency/organization in our services area.

We publicly disclose the methods by which we protect our patient’s rights and the ethical standards by which we conduct business and clinical programs. A copy of specific information may be requested in writing per the HIPAA Regulatory Standards. Santa Rita Home Health will provide information once a written request has been received if disclosure of such information is permitted by law and regulations.

Policy Statement

It is the policy of Home Health to provide the patient (18 years and older) with information regarding their right to formulate Advance Directives, and to know the wishes of the patient or their legal representative regarding the withdrawal or withholding of life sustaining procedures. It is the policy of The Agency to treat all patients equally in the provision of medical care without regard to their execution of an Advance Directive. With the provision of appropriately executed documentation of your advance directive preferences, The Agency staff will provide care and support that is consistent with your choices.

Notice of Nondiscrimination

Pursuant to Title 3 VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, and the Age Discrimination Act of 1975 the Agency does not discriminate on the basis of race, color, nationality origin, disability, diagnosis or age, or other statutes that prohibit discrimination on the basis of sex and sexual orientation.

Patient Privacy and Confidentiality

The patient has the right to know how patient identifiable information is communicated. Home Health periodically examines information handling processes to determine that information requests meet established parameters of the local organization limited to “need to know” and “minimal necessary” guides. Your information is only provided to your identified payer source(s), federal government intermediaries and or regulatory auditors/surveyors and internal clinical, company business office and quality management personnel. In the event that payment is contested, information may be provided to the Administrative Law Judge appeal system, or representatives for the company, all with signed confidentiality statements.
